About personalized case management services
There will be no change to Patient Advocate Foundation’s personalized case management services or CareLines as a result of our merger. Our hope is that we will be able to serve more even people with personalized case management services.
Eligible patients and their families can get free one-on-one case management support from Patient Advocate Foundation. Our case managers help patients with a range of healthcare access needs, such as access to care, understanding insurance, treatment costs, living expenses, and employment.

We encourage you to call us toll free at 800-532-5274, Monday through Friday from 8:30 am to 5:00 pm ET, to answer a few questions and get matched with a case manager who can best meet your needs.
